To seek medical attention promptly.
Provision of Information
To provide complete and accurate information, to the best of your knowledge, regarding your medical history, including past illnesses, allergies,
hospitalization, medications and other matters relating to your health.
Compliance with Instructions
To follow health advice and medical instructions established by your physician and to notify him/her of any significant change or failure to improve after you leave the hospital.
To ask about anything you do not understand relating to the course of your medical treatment.
To keep appointments or cancel 24 hours in advance.
To seek non-emergency care during regular hours.
Refusal of Treatment
You are responsible for the consequences if you refuse treatment or do not follow your physicians, nurses or rehabilitation instructions and for any outcomes of such actions.
Hospital and Professional Charges
To assure that the financial obligations of your healthcare are fulfilled as promptly as possible. You will also provide necessary information and documentation requested by the Neuro Spinal Hospital financial representative.
Respect and Consideration
Be considerate of other patients and see that your visitors are considerate as well.
Too many visitors and excessive noise may be annoying to other patients.
Be respectful of other people’s property and that of the hospital.
Take responsible measures to protect your personal belongings.

PATIENT COMPLAINTS
You and your family may submit a request for information, or an expression of concern, verbally or in writing to the Hospital Administrator.
The Hospital Administrator may be reached at telephone number +9714 315 7709 from 8:00 am to 5:00 pm, Saturday - Wednesday and 8:00 am to 1:00 pm on Thursday.
If at anytime the Hospital Administrator is not available, call the Hospital Manager through the hospital operator at +9714 342 0000 or you may refer the problem, in writing, to the:
